Day Shift - 8am-5pm
Job Description:
Provide counseling and support as needed to residents and their families.
Supervise and assist with pre-admission, admission, and discharge plans for residents, to include obtaining all necessary documentation.
Maintain accurate and timely medical chart documentation on each resident as required by regulation & facility policy and procedure.
Provide appropriate input regarding residents ' care planning; attend and accurately participates in meetings Develop and facilitate support, problem solving, and advocacy groups, including Resident and Family Council Provide staff with resident rights, orientation, and mandatory in-service training.
Works as and assures all department staff work as a member of the NYSVHB team.
Other facility staff view Social Services Department as helpful and supportive of their efforts.
Assists with resident care functions if help is needed.
