Summary:
The Care Coordinator- Social Care assumes a key role within the 8 counties of WNY assisting with and coordinating service linkages to address identified social care needs of participants of all ages.
With focus on comprehensive and integrated social care response for diverse populations/community needs, responsibilities may include a blend of direct case management, outreach and referral, advocacy, and/or volunteer coordination to address a variety of social care needs that range from financial and economic stability, individual and family stability and connectedness, to mental wellness.
Aligned with Catholic Charities' goal of optimum emotional, mental and physical health for all, this team member will provide culturally and linguistically adequate site and/or community-based services to empower native and foreign-born participants to access internal and external supportive resources.

COA, BBB) Maintain professional and competent infusion of agency values in care, and offer quality, participant-centered, strengths-based, solution-focused, trauma-informed care Conduct comprehensive and accurate assessment of presenting social care needs, contributing factors, and participant strengths and existing supports/ resources Utilize person-centered practices to empower participants to develop and monitor Individualized Services Plans (ISP) Provide direct assistance and/or coordinate linkage and referral to applicable resource(s) Schedule and facilitate Individualized Services Plan (ISP) reviews, as needed, including evaluation of advancements and barriers to progress; Revise the plan as appropriate Maintain and concurrently track activities with assigned participants in respective participant record systems and databases Develop and sustain an integrated collateral network of multi-disciplinary provider resources Participate in required staff meetings and designated training.
Perform other duties as assigned.
EDUCATION:
Associates degree in social work, mental health counseling, public health, gerontology or other approved related field preferred.
Bachelor's degree preferred.
EXPERIENCE, KNOWLEDGE, SKILLS AND ABILITIES:
Two (2) years direct experience linking clients to a broad range of essential, community-based services.
Demonstrated professionalism with pleasant and welcoming demeanor and commitment serving with compassion and integrity Demonstrated ability to engage and maintain effective relationships with participants and multidisciplinary teams.
In-depth familiarity with community health, behavioral health and social services systems, organizations and resources.
Ability to increase skills related to interventions necessary to provide effective service.
Demonstrated agility, initiative, ability to work independently and efficiently (effective organizational and time management skills) and produce Exceptional interpersonal and communication (oral and written) skills required.
Demonstrate sensitivity and respect for health disparities, with passion to advocate for and promote health equity- a plus! Fluent in various computer applications (e.
g.
Word, Excel), electronic database systems and electronic health records a plus.
ADDITIONAL REQUIREMENTS:
Requires travel - must have access to reliable transportation, agency-required level of auto insurance coverage, and be able to travel as needed within an assigned service region Must be prepared to work a flexible schedule in order to participate in agency-related activities/events that may take place outside of typical business hours.
